Become a member at Dockside Co-op

In order to become a member of Dockside Co-op, you will need:

- A valid WA state picture ID (driver’s license)
- Your doctor’s recommendation on tamper-proof paper (the ID cards that some clinics give out are not sufficient).
- To fill out a Dockside Co-op application in person.

Please note that the first time you come in we will need to verify with your doctor that you are a patient, so come in when your doctor’s office or clinic is open. Allow about 15 minutes for this process.
